Please read this Agreement carefully before accessing or using the Website. By accessing or using any part of the web site, you agree to become bound by the terms and conditions of this agreement.
This system is restricted to use for legitimate business purposes only. All electronic information transmitted by, received from, or stored in this system is the property of Wake Forest University Health Sciences or the granting institution. WFUHS bears no responsibility or liability for the accuracy or veracity of information submitted. The use of this system may be monitored and recorded for administrative and security reasons. Information submitted through this system will be transmitted to WFUHS where it will be stored and secured in our servers. Access to that data will be limited to approved personnel.
By logging in, you acknowledge the following: that you have read and understand this statement regarding the use of this system; that access or use of this system for fraudulent activity is strictly prohibited by WFUHS and the Federal Government; that use of this system or the reporting of activity which you know to be false, may subject you to criminal or civil penalties/remedies under state, federal, or other applicable domestic and foreign laws.
Privacy Policy
Wake Forest University Baptist Medical Center Division of Public Health Sciences (WFUBMC/PHS) is recognized for our commitment to safeguarding consumer privacy on our web sites. We operate under the following set of strict privacy principles:
Most access of our systems is from individuals doing work on our behalf, but occasionally visitors use our site to obtain various amounts of publically available information.
For Individuals Working on Our Behalf, WFUBMC/PHS does maintain some amount of identifiable information such as name, email address, and phone number. That information is kept strictly confidential and is only made available to approved individuals.
The only information WFUBMC/PHS obtains about Individual Visitors to its web sites is that supplied voluntarily by visitors.
In cases when WFUBMC/PHS may need personal information to provide visitors with customized content or to inform them about new features or services, visitors are explicitly asked for that information.
Personally identifiable information provided by visitors (name, e-mail or home address, etc.) will not be disclosed to anyone.

- IP Addresses
An IP address is a number automatically assigned to your computer whenever you access the Internet. All computer identification on the Internet is conducted with IP addresses, which allow computers and servers to recognize and communicate with each other. WFUBMC/PHS may collect IP addresses in order to conduct system administration and to conduct site analysis. WFUBMC/PHS may also use IP addresses to identify any visitors who refuse to comply with the WFUBMC/PHS Terms of Service agreement, and to identify visitors who threaten our service, site, customers or others.
- Cookies
Our sites may place a text file called a "cookie" in the browser files of your computer. Cookies are pieces of information that a web site transfers to an individual's hard disk for record keeping purposes. Our site uses cookies during your online session to deliver content specific to your interests. Cookies allow us to avoid showing you the same message repeatedly. Our cookies do not contain personal information. They do enable WFUBMC/PHS to relate your use of the site to information that you have specifically and knowingly provided to our site.
- Surveys and Questionnaires
Our online surveys and questionnaires may ask visitors for contact and demographic information. The customer's contact information is used to contact the visitor when necessary. Users may opt-out of receiving future mailings; see the choice/opt-in section below. Demographic and profile data is also collected at our site but is only used for internal analysis for improving our web sites.
- Security
WFUBMC/PHS web sites have security measures in place to protect the loss, misuse and alteration of the information under our control.
